What are Dissociative Disorders?

Dissociative disorders are a group of mental health conditions characterized by disruptions or disturbances in an individual’s consciousness, memory, identity, perception, or sense of self. These disruptions often involve a dissociation or detachment from one’s thoughts, feelings, memories, or surroundings, leading to a sense of being disconnected or “split off” from reality.

Dissociative disorders often develop as a response to traumatic experiences, such as childhood abuse, accidents, combat, or other overwhelming events. The dissociation serves as a coping mechanism to shield the individual from the full impact of the trauma.

Diagnosing and treating dissociative disorders can be challenging, as individuals may not always be aware of their dissociative experiences. A comprehensive evaluation by a qualified mental health professional, such as a psychiatrist or psychologist, is essential to identify and diagnose these disorders accurately.

DISSOCIATIVE DISORDERS DEMOGRAPHICS

A total of 893 patients had been diagnosed with dissociative disorder over the past decade: 591 (66%) were outpatients and 302 (34%) were inpatients. The proportion of patients diagnosed with dissociative disorders ranged between 1.5 and 15.0 per 1,000 for outpatients and between 1.5 and 11.6 per 1,000 for inpatients. The majority of patients were diagnosed with dissociative motor disorder (43.3% outpatients, 37.7% inpatients), followed by dissociative convulsions (23% outpatients, 27.8% inpatients). Female preponderance was seen across all sub-types of dissociative disorder except dissociative fugue.

SYMPTOMS

The symptoms of dissociative disorders can vary depending on the specific type of disorder, but they all involve some degree of dissociation or detachment from one’s thoughts, feelings, memories, or sense of self. Here are the main symptoms associated with different types of dissociative disorders:

Dissociative Identity Disorder (DID)

   – Presence of two or more distinct personality states or “alters” that control the person’s behavior.

   – Gaps in memory for everyday events or important personal information, often referred to as “lost time.”

   – Alters may have different names, ages, genders, voices, and even physical characteristics.

   – Alters may have varying levels of awareness of each other.

Dissociative Amnesia

   – Inability to recall important personal information, usually related to a traumatic event or stressful experience.

   – Memory loss is more extensive than ordinary forgetfulness and cannot be explained by a neurological condition.

   – The memory loss is often selective and may involve specific periods or aspects of the individual’s life.

Depersonalization-Derealization Disorder

   – Depersonalization: Feeling detached from oneself, as if observing oneself from the outside or in a dream-like state.

   – Derealization: Experiencing the external world as unreal, foggy, or distorted.

It’s important to note that dissociative symptoms can also be present in other mental health conditions, such as post-traumatic stress disorder (PTSD) or acute stress disorder. Therefore, a comprehensive evaluation by a qualified mental health professional is necessary to accurately diagnose dissociative disorders and differentiate them from other conditions.

In some cases, individuals with dissociative disorders may also experience co-existing symptoms, such as depression, anxiety, self-harm, or suicidal thoughts. Additionally, they may have a history of traumatic experiences or childhood abuse.

COUNSELING

Counseling for dissociative disorders is an essential aspect of treatment, aiming to help individuals with dissociative symptoms understand, manage, and integrate their experiences. Therapists use trauma-focused approaches, such as Trauma-Focused Cognitive-Behavioral Therapy (TF-CBT) or Eye Movement Desensitization and Reprocessing (EMDR), to address underlying traumatic events. The focus is on creating a safe and supportive therapeutic environment, promoting communication between dissociated parts, and developing coping skills to manage dissociative episodes. Therapy aims to foster integration, reduce distress, improve functioning, and help individuals develop a cohesive sense of self. Collaboration with other mental health professionals may be necessary to provide comprehensive care for individuals with dissociative disorders.
